Consultations

Process from reception to payment

    Go to reception

    When you arrive at the hospital, please go to the counter #1 in the central waiting hall on the left-hand side of the main entrance.

    Fill out the consultation application form

    You will be asked to fill out the form using our machine translation tool.

  • 3
    Fill out the questionnaire

    Please fill out the questionnaire in a language you can understand (English, Chinese, Korean or Tagalog). For other languages, you will be asked to fill out a Japanese language questionnaire using the hospital's machine translation tool.

  • 4
    Go to the outpatient area, and undergo an examination

    A staff member will show you the floor map and guide you to the outpatient area.

  • 5
    Pay the fee
    • When you finish your examination, please submit your medical records at counter #4 in the central waiting hall.
    • When the cost of your treatment is calculated, you will return your medical examination ticket and other documents at counter #4.
    • Please show your examination ticket at counter #5 and pay the fee.
    • You can pay the medical fee either by cash in Japanese currency or using a credit card.
